Meet Tristan M. Brooks, LCSW

Founder of Chosen Path Wellness

Hello, and welcome.

I'm Tristan Brooks, a Licensed Clinical Social Worker (LCSW) and the founder of Chosen Path Wellness. I believe healing begins when people feel safe enough to show up as their authentic selves. My goal is to provide a supportive, affirming, and nonjudgmental space where you can explore life's challenges, develop new perspectives, and move toward meaningful growth and lasting wellness.

With over six years of experience in the mental health field, I have worked with adolescents, young adults, adults, and families across a variety of settings, including inpatient psychiatric units, residential and group home programs, detention facilities, schools, and outpatient therapy. These experiences have given me the opportunity to support individuals facing a wide range of emotional, behavioral, and life challenges while developing a deep appreciation for the resilience people carry, even during their most difficult moments.

I earned my Master of Social Work degree from Rutgers University and utilize evidence-based approaches including Cognitive Behavioral Therapy (CBT), trauma-informed care, behavioral interventions, and strengths-based treatment. My approach is collaborative, practical, and personalized. I don't believe therapy is about "fixing" people. Instead, I view therapy as a partnership focused on helping you identify your strengths, build effective coping skills, and create the life you want for yourself.

I have a particular passion for working with LGBTQIA+ individuals, BIPOC communities, and those navigating anxiety, depression, relationship concerns, identity development, stress, trauma, and life transitions. I am committed to creating a culturally responsive and affirming environment where every client feels respected, understood, and empowered.

At Chosen Path Wellness, I believe that each person's journey is unique. Whether you're seeking support during a difficult season, working through longstanding challenges, or striving to become the best version of yourself, I am here to walk alongside you. Together, we can explore what has brought you to this point, build on your existing strengths, and develop tools that support healing, confidence, and personal growth.

Outside of my work as a therapist, I'm a proud dog lover and someone who values authenticity, connection, and lifelong learning. These values shape both my personal life and my work with clients. I understand the courage it takes to ask for help, and I consider it a privilege to support individuals as they navigate their path toward healing and wellness.

I look forward to the opportunity to walk alongside you on your journey.
